We are currently seeking a highly skilled Telephone Triage Nurse to join our team. This vital role plays a critical part in ensuring the well-being of our patients by efficiently providing triage telephone service to ensure prompt identification of patients with high-risk conditions.

Responsibilities:

  • Engage in telephone consultations with patients, offering guidance on the appropriate course of action, which may involve directing them to emergency rooms (ERs), urgent care centers, or their primary care physicians.
  • Conduct concise assessments of patients' conditions to gauge the severity of their illness or injury, including gathering information about their primary complaints and, when possible, collecting vital signs and mental status information.
  • Determine the urgency of seeing the patient based on a brief assessment and familiarity with a patient’s condition and history. May use a computerized medical information database, which uses algorithms that closely imitate physician logic and thought patterns, as a guide. Consult with a physician as necessary.
  • Immediately refer patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to the ER or arrange for ambulance transportation. May Provide appropriate home health advice to individuals whose conditions do not warrant an immediate ER visit.
  • Coordinate appointments for patients who require medical attention but do not necessitate an ER visit, either directly or by arranging for an appointment scheduler to make the appointment.
  • When designated, function as an "Ask a Nurse" resource, handling routine inquiries from patients, such as, "Do I need an annual flu shot? When are these vaccinations available?"
  • Other duties as assigned.

Requirements

Basic Qualifications

  • Valid driver's license.
  • RN degree; Current state RN license. Current CPR certificate. On-the-job training in triage.
  • Minimum five years of experience as an RN; one to two years of telephone triage experience.
  • Ability to communicate clearly and calmly with patients.
  • Ability to work closely with physicians and other clinicians as needed.
  • Knowledge of telephone-based clinical assessment techniques.
  • Knowledge of medical practice telephone triage protocols.
  • Knowledge of home health guidelines for patients to follow until their scheduled physician visit, in cases where an immediate ER visit is not required.
  • Ability to use electronic medical records to check patient history
